How to Prepare For Your First Appointment

After making an appointment, please download the intake form (below), fill it out, and bring it with you to your first appointment. Also, remember to bring any insurance information with you. This includes your card and authorization number if applicable. If you have any questions, please use the contact form on the left. We will answer as soon as possible. Please plan to arrive a few minutes early so we can start on time. We want to give you as much time as possible.

Payment is expected at the end of each session. We accept check, cash, and major credit cards.
